THE CHANGING LIVES COMMUNITY FUND INAUGURAL GRANT CYCLE AWARDEES

On behalf of the board of directors of Nevada Community Foundation, we would like to thank the Changing Lives Community Fund for its generosity and for making a positive impact in southern Nevada.

 

A kind-hearted man, Stan Tomchin, brought the Changing Lives Community Fund to life. His generous giving spirit and determination to help people has created a positive pathway for change. His vision is to help the un-met needs of the Las Vegas community by serving as a catalyst to make a difference in the lives of others. The Changing Lives Fund will help improve Nevada’s health, education, arts and quality of life sectors. The fund will provide resources necessary for addressing Nevada’s most crucial issues to effect social change and care in the community. 

 

This funding cycle is targeted to organizations focused on meeting unmet basic human needs and improving the well-being of women and children. Support includes but is not limited to: clothing, housing, feeding and caring for women and children in Southern Nevada.

The Fund will award 6-10 grants ranging from $8,000- $20,000.  Please note that this is a one-time grant award for a period of up to one year. 

Proposed projects should:

  • Address an urgent, vital need
  • Demonstrate a unique idea/ proposal to address a crucial problem
  • Engage in community collaboration to encourage change
  • Utilize best or promising practice(s) with data to demonstrate impact


For consideration, the attached grant application must be completed and submitted to the Nevada Community Foundation along with a copy of your organization’s 501 (c)(3) (tax exempt) letter and a list of the Board of Directors. 


All proposals are due to the Nevada Community Foundation on Tuesday, November 29, 2011 by 5 pm.   If an organization is selected for a site visit, the visit will take place between December 5th and December 10th.  


Notification of funding will be made by December 16th, 2011.  


All proposals must include the following components and incomplete or late proposals will not be considered:

  1. Proposal Cover Page (attached)
  2. Proposal Narrative – not to exceed 5 pages (see below)
  3. Budget and Budget Narrative – not to exceed 2 pages
  4. List of Board of Directors
  5. 501 (c) (3) IRS Tax Exempt Letter



NARRATIVE INFORMATION:


Proposals narratives should not exceed 5 (five) pages, single spaced, with a minimum font size of 12. The narrative should include the following components, which must be labeled by section number and title (underlined below):


Section I: Statement of Need – Please succinctly describe the problem or need which this project proposes to address. Include an appropriate description of the population to be served, including race/ethnicity, gender, age, and any other relevant demographic factors.

Section II: Detailed Description of Project/Activities – Please clearly state the goals, objectives and activities that you plan to accomplish with this project.  This section should describe how the proposed project will meet the need provided in the previous section.

Section III: Description of Community Collaboration/Partnerships – Please describe how the applicant agency will engage in partnerships and/or collaborations to accomplish this project, avoid duplication of efforts, and maximize/leverage resources to meet the needs described in Section I.

Section IV: Evaluation Methods and/or Metrics Used to Measure Impact – Provide a detailed description of how you will measure the effectiveness and/or success of this project in meeting the goals and objectives outlined in Section II.  All funded projects must provide a written report to the funder within 90 days of the project end date.

Section V: Organizational Capacity and Key Project Personnel – Describe your organization’s capacity to fulfill the project activities being proposed, including experience and expertise in the work being proposed.  Also include a list of the names, roles and relevant experience of all key project personnel.

Section VI: Project Site and Geographic Boundaries – Describe the site(s) where the project will take place, as well as the geographic boundaries (if any) of the service area.
